Adding, moving, updating equipment is typical for teleport operations. Events and incidents are more likely to occur when changes are made. This section assess a teleport’s procedures concerning managing changes and the associated risks.
a. When must a Request for Change (RFC) procedure be followed?
b. How many people, in what job categories, are involved with approving an RFC?
c. What factors are taken into consideration for assessing the risk of a change?
d. What items are included in an RFC execution plan?
e. Before a change has been implemented, who is notified?
f. After a change has been implemented, who is notified?
g. Is the approval team notified of changes that were not documented in the change request?
h. Are problems and trouble tickets correlated with changes?
i. How often is the change management process reviewed?
j. Is there a well-defined contractor/vendor vetting or accreditation process?
k. Are there health and safety procedures and guidelines which are adhered to when working with high voltage equipment?
